I would prescribe Celebrex, 200mg a day by mouth for his joint pain. Celebrex is a nonsteroidal anti-inflammatory that inhibits cyclo-oxygenase-2, the enzyme responsible for producing prostaglandins that cause pain and inflammation, producing anti-inflammatory effects. This in turn would provide relief of the pain. This drug is contradicted in clients who have a hypersensitivity to sulfonamides, NSAID’s and aspirin. It also interacts with warfarin, lithium and fluconazole. I would advise the client to take with food if GI upset occurs and to avoid taking aspirin and alcohol while taking this drug. The frequent side effects are diarrhea, dyspepsia, headache, and abdominal pain. For the clients pneumonia he would be prescribed Levaquin 500mg daily. Levaquin is a Fluoroquinolone class antibiotic. This is a wide spectrum antibiotic used in the treatment of pneumonia and also for the treatment of acute exacerbation of chronic bronchitis. It inhibits the DNA enzyme, gyrase, in susceptible microorganisms, interfering with the bacterial DNA replication and repair, producing bactericidal activity. It interacts with antacids, sucralfate, iron preparations decrease levofloxacin absorption. NSAIDs may increase the risk of CNS stimulation and seizures. It may also alter blood glucose levels. I would advise the client not to take antacids within two hours of the levaquin and can be taken without regards to food. The client may have side effects such as diarrhea, nausea, stomach pain, dizziness, drowsiness, headaches and light-headedness. The pneumonic process may involve primarily the interstitium or the alveoli. Involvement of an entire lobe is called lobar pneumonia.
